
Our easy-to-understand classes are designed to help you learn about your diabetes and how to manage it day-to-day. You’ll get useful tips on eating well, being active, checking your blood sugar, and staying healthy. Our program includes a personalized one-on-one consultation with a diabetes educator followed by four group class sessions. This program is recognized by the American Diabetes Association for meeting the National Standards for Diabetes Self-Management Education and Support (DSMES). This service is available only through a referral from your healthcare provider who manages your diabetes. This program is paid for by Medicare and most insurance plans. If you have diabetes education benefit questions, please contact your insurance company. Your first one-on-one visit is billed as G0108. Group classes are billed as G0109.
