Medication Management Program
Taking care of your heart is one of the most important things you can do to live a long and healthy life. Blood thinners offer protection from the development of a blood clot and protects against heart attack and stroke. These medications are often used if you live with a clotting disorder, have deep vein thrombosis or pulmonary embolism, have undergone heart valve surgery or other complicated surgical procedures, or if you have an irregular heart rhythm called atrial fibrillation.
WellSpan’s Medication Management Program works closely with you and your primary care provider to coordinate your blood thinner medication plan. Through our program, we provide a seamless approach to medication management and help you improve the quality of your life.
A physician referral is required for the Medication Management Program.
How We Can Help
The following services are available through the WellSpan’s Medication Management Program:
- in-depth, one-on-one, education by a cardiac registered nurse
- finger stick INR testing to measure the effects of Warfarin (Coumadin, Jantoven), eliminating the need for blood drawn from the vein
- proper dosage adjustment and recommendations based on blood work results
- thorough ongoing educational services
- close follow-up
