Memory Care Evaluation and Planning

WellSpan Geriatrics offers comprehensive evaluations for patients 65 years or older with memory related concerns. Our team of caring, board-certified geriatricians along with our licensed clinical social worker will complete an extensive evaluation and diagnosis.

We encourage a loved one to attend appointments, so information and education can be shared correctly. After the evaluation, our geriatricians will discuss treatment options depending on the forms of dementia and/or cognitive impairment diagnosed. A care plan will be put in place and allow for each patient to live their fullest life.

Our practice provides specialized geriatric evaluations while working closely with your primary care provider to improve your health and wellness. If you don’t have a PCP, we would be happy to refer you to one of our providers. Our geriatric specialists’ practice is specific to cognitive impairment assessment and treatment.

How We Can Help

The geriatric team at WellSpan can help evaluate and treat patients 65 years or older for dementia or cognitive impairment related to dementia. This evaluation of the patient consists of two appointments.

1. Assessment

With a Social Worker
Lasts one hour
Cognitive testing
Gathers additional information for physician related to dementia
Community resources and referrals can be provided, if needed

2. Exam

With a geriatrician
Lasts one and a half hours
Scheduled about 2-3 weeks after the assessment
Medical testing such as vision, hearing and blood pressure screening

After an evaluation, patients will be referred to agencies for additional community support.

Additionally, our geriatric team provides support as attending physicians and advanced practice clinicians to residents at some skilled nursing and assisted living facilities in York County.
