Do you have post-partum depression with severe depression or harmful thoughts?

Are you experiencing heavy vaginal bleeding?

Do you have a diagnosis of hypertension from your PCP?

Are you on three or more medications for hypertension?

Were your last two blood pressure recordings equal to or greater than 170/100?

Is the patient over 18 years of age?

Is this for surgical clearance?

Is this for insomnia?

Is this for narcolepsy?

Is your ingrown toenail draining or infected?

Is your ulcer draining or infected?

Was your fracture within the last 14 days?

Is your wound draining or infected?

Do you have very large breasts that cause you back or neck pain?

Is the goal to make your breasts smaller?

Do you have a large amount of excess skin on your lower abdomen?

Is the excess skin due to a massive weight loss 50 pounds or greater?

Do you have an abnormal growth or appearance on your skin compared to the skin around it?

Have you noticed any abnormal changes including increased size, change in color, pain, bleeding, cracking, or oozing in your skin lesion?

Has the spot been present for more than 4 weeks?
