WellSpan Bridges to Health is a special, invitation-only program developed for patients who have complex healthcare needs and who require more support than a traditional primary care doctor can typically provide.
While enrolled in the program, patients work with a dedicated health care team made up of a doctor, nurse, social worker and RN Case Managers. Working very closely with each patient, the team develops a customized care plan that focuses on both treatment and on non-medical details that currently are preventing the patient from receiving optimal care. Examples of issues the team may address include:
- Transportation to and from healthcare appointments
- Extra assistance navigating local health care systems and community-based support programs
- Evaluation of resources at home (e.g., equipment, family support)
- Other non-traditional primary care interventions
As part of our ongoing goal to make WellSpan services easier to access and use, we will coordinate your care through MyWellSpan, our secure online tool. Use MyWellSpan to manage your appointments, review lab and imaging study results, communicate with your doctor, access your confidential medical records and more.
