For patients who have undergone surgeries or procedures to treat heart disease, it is important to change any unhealthy habits that helped to create the problem in the first place. The Cardiovascular Health Management Program provides care for patients following open heart surgery or a stent procedure or for patients with abnormal heart catheterizations or peripheral vascular disease that require medical treatment.

The following services are available through the Cardiovascular Health Management Program:

  • Integrated approach to aggressive control of risk factors
  • Diet assessment and education and resources for recommended changes
  • Counseling and education to promote a heart healthy way of life
  • Special cholesterol and lipoprotein testing
  • Development of an action plan including lifestyle changes and medications
  • Referrals, if needed, to cardiac rehabilitation, diet counseling, smoking cessation program, weight loss program, and stress management program

Referrals

The referral to this program will be part of the discharge process. The appointment will be scheduled before the patient leaves the hospital.

About the provider search

This search will provide you with WellSpan Medical Group and Northern Lancaster County (Ephrata) Medical Group primary care physicians and specialists. If we don’t have a WellSpan Medical Group physician to meet your criteria, the search will expand to include community physicians who partner with WellSpan Medical Group physicians through the WellSpan Provider Network or provide care to patients on the Medical Staffs of WellSpan’s Hospitals.
