The Horizon Planning area of MyWellSpan allows you to view documents WellSpan has on file, such as advance directives, living will, and power of attorney, and access resources to help with care planning.

You can also upload documents that you have created. If you do not have an electronic version of the document available to upload, you can use your smartphone to scan the document so that you can upload the file.
