21st Century Cures Act Intake Form

The federal government's 21st Century Cures Act Final Rule is legislation that supports seamless and secure access, exchange, and use of electronic health information. The final rule is designed to give patients and their healthcare providers secure access to health information.

WellSpan patients can access and request their health information using MyWellSpan, our patient portal. MyWellspan allows patients to view labs imaging and visit notes, manage appointments, communicate with their doctor and more.

In support of the Cures Act Final Rule, the form below allows for other requests to access, exchange or use electronic health information. Requests will be reviewed according to all mandated privacy regulations, and a response will be issued within 10 days.
