Description:

This outpatient education program for people with diabetes provides essential day-to-day skills for better blood sugar control. The program includes 5 group sessions. Individual consult with a registered nurse or a registered dietitian will address all aspects of living with diabetes. A support person is welcome to attend with you. The American Diabetes Association recognizes this program as meeting the National standards for diabetes self-management education. Fees are covered all or in part by Medicare and most health insurance plans. You are responsible to check your insurance coverage.
