Our approach to hospital at home care

Managing a chronic condition or coming home from a hospital stay significantly affects you and your loved ones’ routines.

We offer care at home as a convenient, cost-effective solution. We help you safely settle back into your everyday life.

Our advanced practice providers (APPs), doctors and nurses create a personalized plan that meets your unique needs — pairing hospital-level care with the comforts of home. During your hospital stay, your physician will determine if you are a candidate for this program and will work with the care team to coordinate care at home.

Conditions we treat

Patients hospitalized for low-complexity conditions, such as those below, may be eligible for WellSpan Hospital at Home:

  • Acute exacerbation of asthma
  • Acute exacerbation of chronic obstructive pulmonary disease (COPD)
  • Decompensated congestive heart failure (CHF)
  • Urinary tract infection (UTI)
  • Pneumonia  
  • Cellulitis
  • Deep venous thrombosis (blood clot) (DVT)
  • Pulmonary embolism (blood clot in lungs) (PE)
  • Hypertensive urgency  
  • Diabetes
  • Dehydration 
  • Covid
  • Status post laparoscopic gastric sleeve

Daily visits and check-ins

Nurses will visit you once or twice a day. They'll track your health, give medication, and collaborate with your broader care team. You'll meet daily with your provider, virtually or in person.

Remote patient monitoring (rpm)

RPM is a way to prevent avoidable hospital admission and increase your knowledge to better manage your disease state. Our specialized team of registered nurses monitors your blood pressure, oxygen saturation, weight and heart rate via Bluetooth technology. You don’t need to be enrolled in Hospital at Home to benefit from this service.

Around-the-clock availability

Providing peace of mind and quick response times, we’re always on call for any urgent situations that may arise.

Gradual goodbyes

When you’re ready for discharge from Hospital at Home, we’ll continue to support you through our RPM program for at least 30 days to facilitate your smooth transition back to your everyday life. We’ll also work with you to coordinate any additional services needed, such as therapy or home care.
