That's why we have established relationships with a variety of post-acute care providers throughout central Pennsylvania that are part of our Post-Acute Provider Network (PPN). Working in concert with WellSpan doctors, nurse practitioners, care coordinators and social workers, we strive to make transitions smoother and improve outcomes, offering support during this important phase of care.
Participants in our PPN share our core values and focus on improving care and recovery after a hospital stay. Our team works closely with nursing teams in these facilities to provide both routine and acute care—from supporting physical, mental and emotional changes to providing wellness visits and preventive screenings and managing chronic conditions. We continuously evaluate our PPN facilities on a variety of criteria, including overall quality, cost and outcomes to ensure they meet our quality of care.

Omega Bed Partner: These facilities are special programs partners and participate in the Omega Bed program. Your care team will notify you if you qualify for this program.
WellSpan Medical Director: These facilities have a WellSpan Medical Director and dedicated nurse practitioners to oversee your care.
SNF Waiver Partner: These facilities are special programs partners and participate in the Post-Acute Provider Network 3-Day Waiver program. Your care team will notify you if you qualify for this program.
Top Performer: These facilities have met certain criteria pertaining to quality, cost, outcomes and engagement that make them a top performer.
