Bridges to Health


Why was I invited to be a part of this program?

Each team member at Bridges to Health helps to create every patient’s care plan. This means we can only accept a small number of patients into this new program. We look for patients whose current health care needs are greater than what regular primary care systems can meet. We also look for those who would most benefit from the program’s special services.

If you would like to play a more active part in your own care and be able to reach your health care team anytime you need them, Bridges to Health is the program for you.

How does Bridges to Health work?

The Bridges to Health team will serve as a bridge between you, your doctors and the hospital. Our team is small so you will get to know all of us during your first few visits.  Since we only accept a small number of patients into our program, our team will get to know you, your wishes and your health care needs.

We will work with you to find gaps in your current health care and help you bridge those gaps. We will be here for you for scheduled and walk-in appointments during normal business hours. We will provide you with emergency after-hours access to a member of our team. If you need to talk with us but can’t come to our office, we can answer your questions over the phone or through web-based chats. We will even come to your home if you need that.

For as long as you are part of our program, the Bridges to Health doctor will become your primary care doctor. If you need to see other health care specialists, this doctor will help you see them. The Bridges to Health team will make sure we all talk to each other about your care. We will help you understand the services and types of care you may receive.

With your permission, the Bridges to Health doctor will tell your former primary care doctor about your progress. This will help when you are ready to return to your previous doctor. It will help staff to pick up where we left off.

About the provider search

This search will provide you with WellSpan Medical Group and Northern Lancaster County (Ephrata) Medical Group primary care physicians and specialists. If we don’t have a WellSpan Medical Group physician to meet your criteria, the search will expand to include community physicians who partner with WellSpan Medical Group physicians through the WellSpan Provider Network or provide care to patients on the Medical Staffs of WellSpan’s Hospitals.

New to this practice?

If you don't have a WellSpan primary care provider and would like to schedule a new patient appointment with a provider who is accepting patients, just log into your MyWellSpan account, and go to the Appointment Center section. As you progress through the scheduling process, you will be able to see the offices that are accepting new patients in relation to your zip code. If you are not enrolled in MyWellSpan, go to https://my.wellspan.org, call 1-866-638-1842 or speak with a member of the staff at a participating facility to sign up. New patient scheduling not available at all practices/programs.

Already a patient at this practice?

If you already have a relationship with a WellSpan practice, simply log into your account, and go to the Appointment Center section. As you progress through the scheduling process, you will be able to schedule an appointment with any provider or practice that already counts you as a patient. Online scheduling varies by practice/program.
