Program increases coordinated care between hospital and nursing facility after successful pilot
The York County Board of Commissioners and WellSpan Health expanded an agreement that will benefit more county residents through coordinated care between WellSpan York Hospital and Pleasant Acres Nursing & Rehabilitation Center.
The new agreement bolsters an existing partnership that improves the timely discharge of York County residents from WellSpan York Hospital to a long-term, post-acute care facility.
“This expanded partnership is vital to allow more people who need supervised rehabilitation before they can return to their homes,” said Susan Byrnes, president commissioner of York County.
Under the new yearlong collaboration, WellSpan York Hospital will lease up to 10 resident beds at Pleasant Acres, the county’s skilled nursing facility. This provides a short-term, cost-delayed transition setting for patients at the hospital who no longer need hospital-based care but face obstacles to timely and appropriate hospital discharge.
The program began last year, with the hospital leasing up to six beds at Pleasant Acres. Expanding to 10 beds reflects the program’s success. In its first year, 34 York County residents transitioned from York Hospital to Pleasant Acres.

Through the collaboration, patients now have a short-term option – for up to 90 days – where they can continue to receive safe, coordinated care at Pleasant Acres. WellSpan York Hospital will continue to provide funding for a social worker and a Pleasant Acres clinical liaison to collaborate on patient care and ensure smooth transitions between the hospital and nursing facility.
Additionally, the hospital will provide funding for a part-time case manager. It will also continue providing administrative oversight, case management support, access to clinical programs and health information needed to support the collaborative initiative.
