34 WellSpan Medical Group practices achieve highest recognition for patient-centered care

Growing number of practices in Adams and York counties awarded top level of recognition

Thirty-four WellSpan Medical Group practices in Adams and York counties and northern Maryland have recently been recognized by the National Committee for Quality Assurance (NCQA) for their ongoing commitment to patient-centered care.

These new and renewed three-year designations represent WellSpan’s continued focus on the Patient-Centered Medical Home (PCMH) model that aligns care around the patient, communicating and coordinating across the health system –  from primary care, hospitals and behavioral health, to urgent care.

All 34 practices attained the Level 3 status, which is the highest awarded.

“The whole premise for the medical home model is to provide a foundation for integrated, coordinated patient-centered care in our practices” said Guy Sneed, D.O., vice president and chief medical officer, WellSpan Medical Group. “It establishes the framework for continuous improvement in the quality of care we provide to our patients every day.”

In total, 30 WellSpan primary care practices have received the PCMH recognition, and four WellSpan specialty practices have received the Patient-Centered Specialty Practice (PCSP) recognition. This marks the first time WellSpan specialty practices have received the PCSP designation.

To achieve the “patient-centered” designation, practices must adopt techniques that combine teamwork and information technology to improve quality of care and the overall patients experience, while also reducing costs.

Practices also must show that care is coordinated; that patients are partners in care decisions and have better access through expanded hours; and that medical teams help prevent health problems and manage chronic conditions, among other requirements.

The WellSpan Medical Group has been steadily expanding its patient-centered care model, growing from 28 PCMH practices in 2014.
