Working With Partners to Strengthen Our Communities

We believe good health is not simply the absence of illness and disease, but a balanced state of physical, mental, and social well-being. It is a goal that cannot be achieved alone. Through key partnerships, such as Healthy Community Network, Community Health Council of Lebanon County, Healthy Adams County, Healthy Franklin County and Healthy York County Coalition, WellSpan works with community members, thought leaders, businesses, and health and social service organizations to share ideas and focus on programs and services that will sustain and strengthen our communities.
WellSpan continues to take big steps towards screening and supporting patients with social determinants of health barriers. Social determinants of health are the conditions in the environment where people are born, live, learn, work, play, worship, and age that impact their health, functioning, and quality of life. Improving these conditions can have a bigger impact on health than the medical care they receive. In 2021, about 200,000 patients were screened for food, housing, and/or transportation insecurity. Our care management team takes swift action to meet any urgent needs with charity care funds, while connecting eligible patients to social services and enrolling them in special programs across the community and within WellSpan.
