Comprehensive Women's Heart Risk Assessment
The Women's Heart Program offers a free heart risk assessment which will help you to assess your overall heart health. Your risk assessment helps us to pinpoint individual risk factors for heart disease and will form the basis for your unique heart disease prevention plan.
Once you complete and submit the assessment, we will contact you to discuss your results, and if necessary, make arrangements for a consultation and a heart disease prevention plan. (You are not obligated to come in for a consultation). If you were referred to our program by your physician, the results of your risk assessment will also serve as a guide for follow-up care.
You may complete the free risk assessment online. You may also call the WellSpan HealthSource at (800) 840-5905 to request a hard copy.
Our nurse educator will provide a report of your consultation to your health care provider and offer recommendations for your follow-up care. In conjunction with your primary care provider, the Women's Heart Program will allow you to more effectively control your risk for heart disease and stroke.
During your consultation, you will be instructed in healthy lifestyle changes that will help prevent or slow the progression of heart disease. Depending on your unique needs, referrals/recommendations may include the following:
- Stress management techniques
- Tobacco cessation services
- Exercise prescription
- Nutrition counseling
- Healthy cooking classes
- Complementary medicine programs/services
- Diabetes management
