39 N. Fifth Street Our hospice program enables terminally ill patients to remain in their residence and to focus on the quality of life. We feature an interdisciplinary team of nurses, home health aides, social workers, medical director, spiritual caregivers and volunteers who work in conjunction with the patient's physician. Our skilled and compassionate professionals strive to preserve the dignity and personal choices of each patient. Services include:
Service HoursOur staff is available 7 days a week, including holidays, and works with each patient to develop a visit schedule that accommodates their needs and wishes. A 24-hour telephone service permits families and patients to discuss care when it is most needed. EligibilityAll residents of York and Adams County are eligible for care regardless of race, color, creed, sex, age, national origin, handicap or ability to pay. A patient may enter the hospice program once he or she ceases active treatment for a cure and focuses on comfort measures. To qualify for hospice services, the patient's life expectancy is measurable in months. ReferralPatients may be referred to VNA Hospice by anyone. There are no age restrictions for patients.
