If the natural passage of time is taking its toll on someone dear to you, it's time to consult the WellSpan Center for Aging. Here, we specialize in the unique healthcare needs of maturing adults. Some of these changes are a direct result of the aging process. However, some could be symptoms of underlying serious medical conditions. Whether your loved one's physical changes are affecting the mind or body, the Center for Aging can determine the cause and best approach for treatment. Talk to your family physician about a referral to the WellSpan Center for Aging. Our ProvidersAt the Center for Aging, our staff strives to meet your health care needs. Our physicians and staff will be partners in your care and treatment. Caring board-certified geriatricians and specially trained psychiatrists, nurses, and social workers staff the center. They conduct evaluations, assess daily living skills, provide diagnosis, treatment, and referrals, and work with families to improve quality of life. From changes in eyesight and hearing loss to Alzheimer's Disease and arthritis, the Center for Aging has the resources to help adults age gracefully and healthfully. The Geriatric AssessmentAt the Center for Aging, our board certified geriatricians will conduct a geriatric assessment, which is a comprehensive evaluation designed to assess changing physical and mental health. The goal of the assessment is to improve overall quality of life, reduce the need for hospitalization and/or institutionalization and enable the patient to live independently for as long as possible. By conducting a comprehensive evaluation, our experienced specialists will be able to plan the correct course of care. We will look at the following:
The Assessment Process At the first physician office visit, the healthcare team will conduct a comprehensive evaluation that includes a complete medical and social history, mental status testing, physical examination and nutritional review. This appointment is scheduled for one hour. Please arrive at least 15 minutes prior to the appointment. The team may suggest other diagnostic tests that may reveal the cause of symptoms including blood work, x-rays and brain imaging (MRI or CT scan). The doctor may also suggest consultation with other medical specialists, such as physical therapy. A home safety evaluation may also be requested. Approximately two weeks later the patient will be scheduled for a family conference. At the family conference, the physician will discuss the findings with the patient and caregivers and review the diagnoses and recommendations for treatment. Families receive practical suggestions for the care of their loved one that will hopefully improve the patients' and families' quality of life. Upon completion of the assessment, the patient may be asked to attend follow up appointments at 3 to 6 month intervals. Following the evaluation, the Center for Aging will supply the referring physician with a written summary of results and recommendations. PrescriptionsRequests for prescription refills should be made during regular office hours. Insurance and FeesCenter for Aging has established a fee schedule that we will be willing to discuss with you at any time. You are responsible for paying all fees, including co-payments, at the time of your office visit. We participate in many health plans, including SOUTH CENTRAL Preferred, Pennsylvania Blue Shield and many others. Please call our office if you would like to find out if we participate in your plan. We participate in Medicare, however we will bill you for any balance remaining after Medicare payment. Other insurance companies may allow you to have them send payment directly to our office.
