The WellSpan Patient Help Fund is a tool to help patients, often overwhelmed by their circumstance and health issues to take the first step in managing their health.
It's a compassionate response to aid patients with the greatest needs as they transition home from the hospital or outpatient facility after an illness or injury. The fund is used as a bridging tool to help a patient better engage in managing his/her health. The fund provides:
• Transportation costs to and from appointments.
• Durable medical equipment.
• Medication co-pays.
• Equipment that will help patients avoid readmission to the hospital.
